Ready your Future Today with the 2017 / 2018 A3 Collaborative

AIMM is partnering with Apexus, the HRSA-designated Prime Vendor for the 340B Drug Pricing Program, and ASHP (American Society of Health-System Pharmacists) to offer the 2017/2018 National A3 Collaborative, a year-long learning experience to help health care providers succeed in the new era of value-based payment programs.

As a member of the 2017/2018 National A3 Collaborative, organizations will participate in a 12-month transformational experience that will establish a new strategic capability. An implementation team will be formed by each organization to rapidly put in place a working medication management program, or to focus on accelerating the scale and spread of one already in place.

List of Participating Organizations for the National A3 Collaborative Spring Cohort

Participating organizations will deliver three results through value-defined initiatives:

  1. Population management: adopt a population health management strategy that demonstrates value to improving health outcomes, providing better care and demonstrating cost effectiveness,
  2. Integrated services: install the clinical pathways that support the integration of primary care and comprehensive medication management services, and
  3. Performance story: produce evidence-based results for quality improvement and strategic business conversations.

Participating Organizations will have access to:

  • Resources and tools to build the business case for an effective CMM strategy focusing on improved health outcomes, better patient care, and cost effectiveness.
  • AIMM’s proven action learning method, which uses effective leadership skills and quality improvement practices to rapidly identify strategies to build, improve processes and deliver CMM services.
  • Individualized support from a program development coach for technical assistance and consultancy on your organization’s CMM value-based initiative.
  • Monthly webinars with national network of peers and access to best practice models for replication.
  • Live learning events* offering an energizing experience with subject matter experts. Harvest learnings from peers who are pursuing a shared vision and urgency to attain transformational goals.
    *A3 Collaborative Participation fee does not include travel expenses to live learning events.

A3 Collaborative Participation Fee: 

Participation in this dynamic, leading edge collaborative network is offered at a cost of $9,000 for the year, plus the cost of team member travel to in-person events. A3 Collaborative partner, Apexus, is offering scholarship funding to organizations to offset a portion of the costs of participating in the learning collaborative. All organizations are invited to apply for a $6,000 scholarship to cover part of the participation fees. Organizations will be responsible for the remaining $3,000 fee, which is invoiced upon notification of a scholarship award. Scholarship and participation fees cover participation for the 2017 / 2018 Collaborative cycle (Fall Cohort), which ends September 30, 2018.

The application process to join the 2017/2018 A3 Collaborative (Fall Cohort) is now open online. The application deadline is September 15, 2017 however, after that time enrollment, scholarship funds will continue to be awarded on a rolling basis subject to availability.  To ensure a place for your AIMM team, interested organizations are encouraged to apply before September 15, 2017.
