About AIMM

The Alliance for Integrated Medication Management (AIMM) is a non-profit organization working to support wide-spread adoption of team-based medication management services into the care of high-risk, high-cost patients suffering from multiple chronic health conditions.

AIMM partners with health care providers, schools of pharmacy, payers, and other stakeholders to drive change in our health care system from the ground up by creating accountability for patient safety and health, improving quality of care, and championing new payment models  to coordinate care for high-risk, and complex patients that can benefit from a comprehensive medication management approach.

AIMM fosters community partnerships to establish health care delivery systems specifically for patients with multiple chronic conditions who need help with adherence and monitoring of their medication regimens.  Teams of health care professionals integrate clinical pharmacy services with primary care services in order to best support patients and providers with coordinating medication use and achieving desired clinical parameters, such as lower blood pressures and target A1C levels.

For patients with serious chronic health conditions, the lack of coordinated care across health care providers – primary care physicians, specialists, pharmacies and emergency departments – can place them at constant risk for health care crises.

According to the Institute of Medicine (IOM), patients with uncoordinated care lack comprehensive medication management and account for more than a third of total health care costs, much of which is avoidable.  The IOM predicts that targeted efforts aimed at these high-risk, uncoordinated care patients could save over $240 billion per year.
